Car Crash Oklahoma City Oklahoma in Oklahoma City, Oklahoma: A U.S. Air Force OA-1K Skyraider II was destroyed on Oct. 23, 2025, after a trainee pilot inadvertently activated the engine fuel shutoff valve instead of the fuselage fuel tank valve during an Initial Qualification Training sortie from Will Rogers Air National Guard Base near Oklahoma City. The aircraft, callsign 'ZORRO 75' and assigned to the 17th Special Operations Squadron under the 492nd Special Operations Wing, lost power and made a forced landing. Both crew members — an active-duty student pilot in the front cockpit and a civilian contractor instructor pilot in the rear cockpit — escaped without injury. The Accident Investigation Board cited task saturation, communication challenges, ineffective crew resource management, and ineffective task prioritization as contributing factors.
