Car Crash Lebanon Ohio in Lebanon, Ohio: A two-vehicle crash on U.S. 22/3 at Middleboro Road in Salem Township, Warren County, Ohio, on June 25, 2026, killed one person and injured two others. The driver of the Toyota was transported by Lebanon EMS to TriHealth Bethesda North Hospital with serious injuries, while the Toyota's passenger, 87-year-old Jerrold Rajewski of Blanchester, was flown by CareFlight to Miami Valley Hospital, where he died. The driver of the Dodge was taken by Hamilton Township EMS to TriHealth Bethesda North Hospital with non-life-threatening injuries. The Lebanon Post of the Ohio State Highway Patrol is investigating the collision.
