Semi Truck Crash South Charleston West Virginia in South Charleston, West Virginia: Jeffrey Simmons, a 63-year-old Iowa resident, was driving a tractor-trailer westbound along I-64 near South Charleston, West Virginia, on June 23, when the vehicle crashed near Exit 54, leaving the tractor-trailer teetering off the edge of a bridge with smoke rising from the front of the truck. The crash proved fatal for Simmons, according to South Charleston Fire Department Fire Chief Virgil White. His dog, Hennessy, who was riding alongside him, survived the crash and was taken to a West Virginia shelter, which later located the dog's original shelter in Iowa and Simmons' surviving family.
